DISCLAIMER AND HOLD HARMLESS AGREEMENT
In
registering to attend or participate in an ITEC class or classes, you as
the registrant certify in registering for the class that you have read
and will abide by the following.
PLAGIARISM
Student
writing assignments, and written responses to discussions and quiz
questions, should be entirely in their own words so as to avoid
plagiarism.
WHAT
YOU NEED TO KNOW BEFORE
YOU
CAN TAKE YOUR STATE LICENSE EXAMINATIONS
As a new entrant to the
HVAC profession, The State of Connecticut requires that, before you may
sit for your license examination[s], you have first registered as an
apprentice with the CT Department of Labor.
Failure to register as an apprentice will mean that you will not
be able to sit for your license exam.
The state Department of Labor (DOL) administers
Connecticut
's apprenticeship program, with advice from a 12-member State
Apprenticeship Council. The council's members represent industry, labor,
and the public. State law allows the council to adopt minimum standards
of apprenticeship and related and supplementary instruction, issue
certificates of completion to apprentices, and encourage employers to
establish programs and individuals to participate in them.
The labor commissioner, with the council's advice, develops the program's
overall work training standards and convenes business and labor groups
to develop craft-specific training programs and apprenticeship terms.
DOL's Office of Apprenticeship Training is responsible for these
activities, for registering program sponsors and individual apprentices,
and for ensuring that registered programs follow all program standards
and terms (CGS §§ 31-22m to -22t; Conn. Agency Regs. , 31-51d-1 to
-12).
REGISTERING
APPRENTICES
Apprentices register through approved employers.
Apprentices must be at least 16 years old. They must pay DOL $25 when
they register and each year until they complete the program and obtain a
journeyman's license (if required for their trade) or withdraw from the
program. Employers must annually pay a $30 sponsor fee for each
apprentice.
When they register, apprentices receive a registration form that contains
the agreement between the employer and apprentice, which spells out each
party's responsibilities. These include the wage progression schedule,
the trade category in which they will apprentice, and credits for
previous experience. Apprentices also receive a monthly journal in which
they must record their work and classroom instruction hours and in which
their supervisor grades their progress.

WAIVER,
HOLD HARMLESS AGREEMENT AND RELEASE OF LIABILITY
In consideration for the privilege of participating in activities with the
Independent Connecticut Petroleum Association and/or the Independent
Connecticut Petroleum Association Education Foundation, and their
agents, servants or employees the undersigned hereby agrees that:
I understand that there are inherent risks involved in participating in
activities with the Independent Connecticut Petroleum Association and/or
the Independent Connecticut Petroleum Association Education Foundation,
its employees, contractors and assigns, including the risk of serious
physical injury or death, whether participating in activities on or off
the property[ies] owned by the Independent Connecticut Petroleum
Association and/or the Independent Connecticut Petroleum Association
Education Foundation, its AGENTS
SERVANTS
OR EMPLOYEES and I FULLY
ASSUME ALL RISKS ASSOCIATED WITH
THESE ACTIVITIES EVEN IF DUE TO THE
NEGLIGENCE OF the Independent Connecticut Petroleum
Association and/or the Independent Connecticut Petroleum Association
Education Foundation, AND
THEIR AGENTS SERVANTS
OR EMPLOYEES.
I, for myself and for my heirs, assigns, successors,
executors, administrators, and legal representatives,
AGREE TO DEFEND,
INDEMNIFY AND HOLD HARMLESS THE Independent Connecticut
Petroleum Association and/or the Independent Connecticut Petroleum
Association Education Foundation, AND
THEIR AGENTS, SERVANTS OR EMPLOYEES from any and all claims,
suits or demands by anyone arising from my use of the Independent
Connecticut Petroleum Association and/or the Independent Connecticut
Petroleum Association Education Foundation facilities and equipment OR
ACTIVITIES ON OR OFF THE PREMISES OWN OR OPERATED BY THE Independent
Connecticut Petroleum Association and/or the Independent Connecticut
Petroleum Association Education Foundation, AND
THEIR AGENTS, SERVANTS OR EMPLOYEES INCLUDING CLAIMS OF NEGLIGENCE ON THE PART OF the
Independent Connecticut Petroleum Association and/or the Independent
Connecticut Petroleum Association Education Foundation, AND THEIR AGENTS, SERVANTS OR EMPLOYEES.
I, for myself and for my heirs, assigns, successors,
executors, administrators, and legal representatives, HEREBY
RELEASE, AND AGREE THAT I
WILL NOT SUE the
Independent Connecticut Petroleum Association and/or the Independent
Connecticut Petroleum Association Education Foundation AND THEIR AGENTS, SERVANTS OR EMPLOYEES for
money damages for personal injury sustained by me while using the Independent
Connecticut Petroleum Association and/or the Independent Connecticut
Petroleum Association Education Foundation facilities and equipment EVEN
IF DUE TO THE
NEGLIGENCE OF the Independent Connecticut Petroleum
Association and/or the Independent Connecticut Petroleum Association
Education Foundation and their agents, servants or employees.
I
HAVE READ THIS WAIVER, HOLD HARMLESS AGREEMENT AND RELEASE OF LIABILITY
AND FULLY UNDERSTAND ITS TERMS. I FURTHER UNDERSTAND THAT BY SIGNING
THIS AGREEMENT THAT I AM GIVING UP SUBSTANTIAL LEGAL RIGHTS. I HAVE NOT
BEEN INDUCED TO SIGN THIS AGREEMENT BY ANY PROMISE OR REPRESENTATION,
AND I SIGN IT VOLUNTARILY AND OF MY OWN FREE WILL.

Acknowledgment:
I
have read and understand this completely filled in eight (8) page
enrollment agreement.
I understand the curriculum I have chosen is approved as an
individualized self paced program.
The completion of course content will vary depending upon the
rate of individual progress.
I have acknowledged, received, and read a completely filled in
copy of this enrollment contract and the school catalog.
I understand and agree to uphold my obligations and
responsibilities described in this agreement.
"I have read the
Basic Electricity Syllabus, understand and accept its contents. I also
understand that all work in this course must be my own and all required
assignments and tests must be completed to receive a passing grade for
this course. I also agree to take all tests, quizzes and examinations as
stated elsewhere in this course syllabus."
You will not receive any grades for this course
unless you have read and agreed to this Syllabus.
